Concept of Piles according to Ayurveda

In Ayurveda Piles is known as “Arsh”, it is a Sanskrit word which means it causes pricking pain in the anus. Ayurveda considers it as a part of digestive disorders. Apart from that which manifest in the anal region,  Arshas has several other locations in the body like pudendum, female genital tract, throat, palate, mouth, nose, ears, eyelids and skin. It is a fleshy mass of variable size, shape and colour.

Symptoms Of Piles/ Haemorrhoids

  • Intense itching around the anus.

Types Of Haemorrhoid / Piles

  • Internal Haemorrhoid – Haemorrhoids on the inside of the rectum. Often stay as unnoticed and diagnosed when it causes bleeding. Sometimes due to physical excretion, it may prolapse and extend out of the anus.
  • External Haemorrhoids – Haemorrhoids on the outside of the anus. They are more painful because of the many nerve endings located on the skin around the anus.
  • Interno-external Haemorrhoids – both internal and external hemorrhoids occur together.

Types Of Haemorrhoids According To Ayurvedic Doshas

  • VATAJA ARSHAS- Pile masses are dry, slightly red, rough with a hard texture. People suffering from these expel hard stools with pain.
  • PITTAJA ARSHAS- Pile masses are blue at their tip, thin, inflamed, bleeding, red/blue or yellow in color, soft and tender to touch. Other symptoms include fever, burning sensation, fainting, and a feeling of thirst.
  • KAPHAJA ARSHAS- Piles masses are white in colour, deep-rooted, stable, unctuous, and round. Other symptoms include itching , diarrhea, loss of taste, improper digestion and heaviness of the head.
  • RAKTAJA ARSHAS: Pile masses have the feature of pitta. During defecation, hard stools with a large quantity of blood are expelled.

Complication Of Piles/Haemorrhoids

  • Strangulated hemorrhoids/piles can cause extreme pain and infections.
  • Rupture of thrombosed hemorrhoids/piles causes bleeding and pain.
  • Anemia due to significant blood loss from chronic hemorrhoids

What is Piles in AYURVEDA?

In Ayurveda Piles are known as “Arsh” and it causes pricking pain in the anus. Ayurveda considers Piles as a part of Digestive disorders. Due to the causative factors, imbalance in any of the doshas (biological energy) associated with blood,  spread out and move in downward direction and produces sprouts of growth in the anal region. Especially in persons who have weak digestive power and who further indulge in causative factors these sprouts increases in size and form pile masses. Arshas is a condition in which a fleshy mass of variable size, shape and colour appears in and around the anus.

How Ayurveda Prevents Piles?

Ayurveda suggests few steps to follow in order to prevent the occurrence of Piles. Suppression of natural urges of body, travelling and sitting in uneven surface, food (improper diet) that aggravate the biological energies (doshas) are to be avoided. Intake of Regular diet (moist and fibrous diet ) and drinks, proper sleep(avoid day sleep), regular panchakarma therapy,  intake of laxatives, etc can prevent the occurrence of disease.

Kerala Ayurvedic Treatment For Piles

In Ayurveda Piles are treated in these four ways- BHESAJA(medicinal treatment) , KSARA (Alkaline cautery), AGNI (Thermal cautery),SASTRA(Surgery).

Based on the signs and symptoms in patients, the treatment modality is selected. Aim of our treatment is firstly to correct the impaired digestive system , then to alleviate the vata(means air- which governs the all movements of body) and reduce the size of pile mass.

Panchakarma And Other Ayurvedic Treatment In Piles

  • Snehana and swedana therapy (application of oil and fomentation) done using specialized medicinal preparation.
  • Avagaha swedana (Sitz-bath) given with the help of water prepared by boiling the leaves of herbs. This helps in reducing the size of pile mass.
  • Rakta mokshana (Bloodletting) done with the application of leeches or using needles, which helps in letting out the vitiated blood.
  • Anuvasana Basti and Niruha Basti-  Medicated enema is given to pacify vitiated Vata, which plays a major role in the pathogenesis of piles. In this case, Basti has to be administered by an experienced therapist with special care.

Ayurvedic Treatment For Piles

Internal medications like Pippalyadya Ghrita, Nagaradhya Ghrita, Takrarista,  Kaishoora guggulu, Triphala Guggulu, Abhayarishta, Dushparshakarishtam, Haritakyadi Rasayanam, Triphalarista, etc are also beneficial in treating Piles.

Diet And Life Style

Vegetarian Diet

  • Green leafy vegetables are best to reduce constipation.
  • Vegetables like carrot, garlic are effective in reducing the bleeding from piles. It can be taken raw or cooked.
  • Increase the intake of salads specially
  • Vegetable soup prepared of dried radish, horse gram, Bael(Aegle marmelos).
  • Green leafy vegetables can be fried with ghee and oil or can be cooked with curd and fried with ghee or can be mixed with cereals and ginger and given to the patients suffering from piles.
  • Juice of Drum stick leaves juice is a good dietary fiber and helps in relieving constipation.
  • Plantain or banana flower is the best medicine to treat bleeding piles

Non vegetarian Diet

Processed meat and non-vegetarian food are not advised in Piles as it causes constipation, indigestion, etc.

If you have taken non vegetarian food, make sure to drink more water and include fibre rich diet.

  • Broth such as chick bone broth is effective in managing the symptoms of piles. Bone broth protein also strengthens intestine wall, detoxifies the intestinal system and reduces inflammation.
  • Meat soup of mutton adding the powders of Piper longum, Himalayan salt and paste of garlic .

Other Diet:

  • Eat more fruits including orange, fig, strawberries, kiwis, bananas, pears, papayas, apples, grapes and mangoes.
  • Increase daily liquid consumption in the form of water, soup, juice, milk(according to the body constitution), buttermilk, etc.
  • Thick gruel prepared by boiling with  Piper longum, Zingiber officinale, Butea monosperma and made sour by adding buttermilk and sprinkling powder of Piper nigrum is useful in people suffering from piles.

Food To Avoid In Piles

  • Intake of processed, frozen and stale food or food products.
  • Refined foods like pastries, packaged and canned foods.
  • Tea, coffee, aerated drinks, and alcoholic beverages.
  • Pickles, potatoes, and root vegetables.
  • Unhealthy fasting, overeating, eating during indigestion and eating incompatible food.
  • Fast foods, spicy and oily food.
  • Milk, Cheese and other dairy products.
  • White bread
  • Chocolate

Natural Ayurvedic Home Remedies For Piles

  • Apply sesame oil on external dry piles followed by warm fomentation (sitting in warm water)
  • Make a paste mixed with equal amount of powdered long pepper and turmeric with milk. Apply this on the piles externally.
  • Eat 4 figs twice a day. Better to be soaked in water prior to eating.
  • Soak anal area regularly in warm water bath for at least 15mintues.  Often recommended two to three times a day.
  • Sitz bath in Triphala decoction.
  • Keep the anal area clean, cleansing gently with warm water. Don’t use soap.
  • Apply ice packs or cold compressors to relieve swelling.
  • Intake of castor oil reduces constipation, helps in decreasing the size of pile mass and pain
  • In take Triphala powder( Emblica officinalis, Terminalia chebula, Terminalia bellerica)helps to reduce constipation.
  • Increase the intake of salads and fruits.
  • Fresh juice obtained Pomegranate can be taken with raw sugar it’s good for bleeding piles
  • Decoction of Mimosa pudica is useful in bleeding piles.
  • A cup of banana flowers, one teaspoon of fenugreek. Blend them both and obtain the juice. Drink morning and evening one glass continuously for two months to treat bleeding piles.

Life Style Adaptation In Pile/Hemorrhoids

  • Drink plenty of liquids.
  • Increase the intake of fibre rich food
  • Wear loose fitting clothes that can support the healing of haemorrhoids.
  • Maintain a healthy body weight.
  • Follow a routine exercise regimen.
  • Eat at regular time .
  • Stress and tension should be avoided.
  • Good sleep. (Minimum 6 to 7 hours of sleep)

Effective Ayurvedic Herbs For Piles

  • Powdered root of Plumbago geylenica (1g) can be taken with buttermilk daily
  • Paste prepared from the leaves of cannabis can be applied over the anus.
  • Lotus stamen can be taken with butter every morning, it is also effective in bleeding Piles.
  • Terminalia chebula along with buttermilk
  • Terminalia chebula along with jaggery taken before food.
  • Indian Senna (Cassia angustifolia) has a good laxative effect and so can be used in case of constipation.
  • Washing the anal area with the water boiled with leaves of Pavetta crassicaulis is useful in pacifying the swelling and pain around anus.
  • Paste of Root of Cissampelos pareira taken along with curd, milk, sugar and water.

Herbal Tea For Piles

  • Herbal tea prepared of Mullein and Dandelion. This can prevent irritation, inflammation and bleeding.
  • Herbal tea prepared of Emblica officinalis  has anti-inflammatory and immuno-modulatory actions.
  • Herbal tea prepared of Terminalia chebula is good to relieve constipation.
  • Herbal tea prepared out of the soothing menthol component of peppermint oil combined with its anti-inflammatory properties helps to relieve the pressure around the anus and make the bowel movements less painful.
